a square knot tied wrong. both crossings go the same way. it leans. it slips. under load it sometimes jams instead of falling apart, which is the worst of the two outcomes: now you must cut it out.
everyone ties one, the first time. children tie them. people in a hurry tie them. shoelaces are mostly granny knots, which is why a bow on a child comes loose by lunchtime. the cure is to take the second crossing the other way.
